Introduction Glycemic control is the most important aspect in management of diabetes mellitus. Methods Data source and search strategy We followed standard guidelines for the systematic review of diagnostic studies [ 16 ] and used the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement Additional file 1 : PRISMA [ 17 ] as the template for reporting the review.

Study selection We reviewed and examined relevant articles that compared the correlation of fasting and 2- h postprandial plasma glucose with HbA1c value for eligibility beginning with titles and abstracts then followed by full text review.

Definitions Fasting and 2 h postprandial plasma glucose were defined as value of plasma glucose measured after overnight 8—12 h fasting and after 2 h of meals respectively [ 1 ].

Methodological quality assessment Clearly described methods of patient selection and characteristics of the included patients, use of predefined good glycemic control cut off points for HbA1c, FBS and PPBS, description on how the index tests FPG, and 2hPPG and the reference standard HbA1c were conducted and interpreted, clear data collection methods and procedures, reported quality assurance methods training of data collectors, pretesting, supervision, calibration, standardization of meals and reporting of lost follow-up were considered as a study quality indicators.

Data synthesis and statistical analysis The descriptions of original studies were summarized using Tables and forest plot.

Limitation Inclusion of different studies from different geographical location, treatment group as well as from different methodological approach would be problematic to end up with particular inferences.

The level of HbA1c at any point in time is contributed to by all circulating erythrocytes, from the oldest days old to the youngest. However, HbA1c is a "weighted" average of blood glucose levels during the preceding days, meaning that glucose levels in the preceding 30 days contribute substantially more to the level of HbA1c than do glucose levels days earlier.

This explains why the level of HbA1c can increase or decrease relatively quickly with large changes in glucose; it does not take days to detect a clinically meaningful change in HbA1c following a clinically significant change in AG.

In the Diabetes Control and Complications Trial or DCCT New Engl J Med ; study of patients with Type 1 diabetes, quarterly HbA1c determinations were the principal measure of glycemic control; study subjects also performed quarterly hour, 7-point capillary-blood glucose profiles.

Blood specimens were obtained by subjects in the home setting, pre-meal, 90 minutes post-meal, and at bed-time. A more recent study sponsored by the ADA, EASD and IDF was designed to better define the mathematical relationship between HbA1c and AG.

The study included subjects with Type 1 and Type 2 diabetes and without diabetes from 10 international centers. Estimated AG eAG was calculated by combining weighted results from at least 2 days of continuous glucose monitoring performed four times, with seven-point daily self-monitoring of capillary glucose performed at least 3 days per week.
