By Warren R. Heymann, MD, FAAD November 17, Vol. When patients ask me about biotin supplementation for their hair or nails, the usual question is what dose they should take, not whether or not it has any value.

Among U. adults, the prevalence of self-reported biotin has increased greatly, particularly among women and older adults, with 7. This commentary will be structured in a question and answer format. Biotin vitamin B7, vitamin H, coenzyme R is a water-soluble vitamin that acts as a coenzyme of four kinds of carboxylase propionyl coenzyme A carboxylase, pyruvate-carboxylase, acetyl coenzyme A carboxylase, methyl-carbamoyl coenzyme A carboxylase in mitochondria, facilitating three major metabolic reactions: carboxylation, decarboxylation, and dehydrogenation.

Recently, it has been demonstrated that biotin has roles in cell signaling and epigenetic regulation. Dietary sources of biotin include organ meats, eggs, fish, sweet potatoes, and almonds. Pregnancy and smoking increase the metabolism of biotin and would require increased intake, although controversial.

Biotin supplementation beyond the recommended daily dose is only rarely necessary, that being in congenital and acquired cases of biotin deficiency.

The congenital causes are the autosomal recessive biotinidase and holocarboxylase synthetase deficiencies. In the latter, the biotin level is normal, but the demand for biotin is significantly increased, that is, relatively deficient.

Examples of the latter are some inherited metabolic diseases with specialized diet requirements, such as phenylpropionuria or propionemia.

If biotin is not added to the formula, biotin deficiency may occur. Raw egg whites contain avidin that binds to biotin, preventing absorption.

This can be prevented by cooking the eggs, which denatures avidin. Administration of certain medications such as antiepileptic drugs notably valproic acid may become biotin deficient. Long-term antibiotics may alter the gut flora, thereby diminishing biotin absorption.

There is only limited evidence that biotin supplementation provides a benefit for hair and nail growth; to date, there are no clinical trials supporting claims that biotin supplementation improves hair and nail growth or quality. In all cases, patients receiving biotin supplementation had an underlying pathology for poor hair or nail growth.

All cases showed evidence of clinical improvement after receiving biotin. In cases of acquired and inherited causes of biotin deficiency as well as pathologies, such as brittle nail syndrome or uncombable hair, biotin supplementation may be of benefit.

However, we propose these cases are uncommon and that there is lack of sufficient evidence for supplementation in healthy individuals. No severe toxicities have been reported with biotin supplementation.

Results may be falsely high or low. Incorrect test results may lead to inappropriate patient management or misdiagnosis. For example, a falsely low result for troponin, a clinically important biomarker to aid in the diagnosis of heart attacks, may lead to a missed diagnosis and potentially serious clinical implications.

The FDA has received a report that one patient taking high levels of biotin died following falsely low troponin test results when a troponin test known to have biotin interference was used. For dermatologists, thyroid function studies are the most likely tests where biotin interference will be observed.

For example, dehydroepiandrosterone sulfate DHEAS , luteinizing hormone LH , follicle-stimulating hormone FSH , tumor markers such as cancer antigen CA , CA and CA and troponine T.

More research is needed, but there is also some evidence that biotin may improve symptoms of peripheral neuropathyand help people with diabetes gain control of their blood sugar. While the use of biotin is generally safe, the vitamin may interfere with some lab tests and medications, so you should tell your healthcare provider if you plan to use it.

During pregnancy and lactation, the need for many nutrients increases. Some studies suggest that biotin deficiency during pregnancy may increase the risk of preterm labor or fetal growth restriction. Evidence suggests that pregnant women may need to take in two to three times the biotin AI to meet demands during pregnancy.

Breastfeeding also increases the demand for biotin. Multiple sclerosis MS is a disease that impacts the central nervous system and causes symptoms like fatigue, difficulty walking, and weakness. Limited evidence suggests that high-dose biotin supplements may help decrease symptoms in people living with MS.

Biotin activates acetyl-coA carboxylase, an enzyme involved in the synthesis of a layer of protein and fatty substances that surrounds nerves known as myelin. In people with MS, there is decreased synthesis of myelin, which is why scientists believe that biotin supplements may benefit those with MS.

A review of three studies involving people diagnosed with MS found moderate evidence suggesting that high-dose biotin supplements delivering at least milligrams mg of biotin per day administered for 12 to 15 months may help improve symptoms in people with progressive MS PMS.

However, the researchers found that there was a high incidence of laboratory test interference in the participants taking high-dose biotin supplements. More studies are needed to understand how biotin supplements impact people with MS. Although research shows that biotin supplements aren't effective for improving the health of the hair, skin, or nails in healthy people, biotin supplements can help improve hair and nail health in people with a biotin deficiency.

Studies show that biotin supplements improve symptoms like hair loss and poor nail growth in people with low biotin levels. For example, a review that included 18 studies found that, in people with acquired and inherited conditions causing a biotin deficiency, biotin supplements improved hair and nail growth.

Although more research is needed, some studies suggest that biotin supplements may help reduce blood sugar and blood lipid levels in people with type 2 diabetes. A review that included five studies and participants found that supplementation with biotin in doses ranging from 1.

Biotin may help improve blood sugar metabolism and reduce the expression of genes involved in the formation of fat cells, which could help lower blood sugar, cholesterol, and triglyceride levels.

Although these results are encouraging, the researchers noted that future studies are needed to fully understand how biotin supplements may benefit the health of those with type 2 diabetes.

Biotin is concentrated in several plant and animal foods, which is why most people who follow balanced diets easily meet their daily biotin needs. Here are some of the best food sources of biotin:. Biotin is found in smaller amounts in foods like spinach, tuna, broccoli, bananas, cheese, oatmeal, and yogurt.

Biotin is commonly found in dietary supplements like B-complex products, multivitamins, and hair, skin, and nails supplements. That said, people who follow restrictive diets low in biotin-containing foods and those who have medical conditions that impact their ability to absorb and utilize biotin from food sources may need to supplement with biotin in order to maintain healthy levels of this nutrient.

Biotin can be taken at any time of day, with or without food. There is no recommended dose for biotin supplements. However, some people, such as pregnant and breastfeeding women, people with alcohol abuse disorder, and those with conditions that impact their ability to absorb biotin from food, may require a biotin supplement.

For example, studies have found that doses of mg per day, nearly 7, times the AI, did not produce symptoms of toxicity. The UL refers to the maximum daily dose of a vitamin unlikely to cause harm in the general population. High-dose biotin supplements can cause falsely high or falsely low results, which can result in the misdiagnosis of diseases.

However, certain medications have been shown to decrease biotin levels. For example, long-term use of anticonvulsant medications like carbamazepine, phenytoin, phenobarbital, and combinations of these medications may deplete biotin in the body. Some supplement manufacturers hire third-party labs to test their products for purity and potency, which helps improve supplement safety.

Whenever possible, purchase supplements from brands certified by organizations like UL, USP, and NSF International. Lastly, unless specifically recommended by a healthcare provider, high-dose biotin supplements should generally be avoided by most people as they can interfere with common laboratory tests.

There is currently no UL set for biotin as it has low toxicity. Unless your healthcare provider recommends high-dose biotin to treat a specific health condition, you should avoid taking biotin supplements that provide more than the DV, which is set at 30 mcg per day.

Most people get enough through their diet, so biotin deficiency is rare. However, biotin supplements may be appropriate for some people, including pregnant and breastfeeding women and people with certain medical conditions, such as MS and type 2 diabetes.

Cree BAC, Cutter G, Wolinsky JS, et al. Alcoholism can increase the risk of biotin deficiency and many other nutrients as alcohol can block their absorption, and also because alcohol abuse is generally associated with a poor dietary intake.

About a third of pregnant women show a mild biotin deficiency despite eating adequate intakes, though the exact reason is not clear. No evidence in humans has shown a toxicity of biotin even with high intakes.

Because it is water-soluble, any excess amount will leave through the urine. There is no established upper limit or toxic level for biotin. Those who frequently enjoy raw eggs in recipes for mayonnaise, Caesar dressing, or eggnog may want to reconsider. A protein in raw eggs called avidin can bind to biotin, preventing its absorption.

Cooked eggs are not an issue because avidin is broken down when heated. B Vitamins Vitamins and Minerals. A handful of case reports and small trials have shown a benefit, but the study designs had weaknesses: The diagnoses of the type of hair conditions varied or were not cited at all. Researchers have also noted that certain hair loss conditions like alopecia can resolve spontaneously without treatment, so it is not clear that biotin supplements specifically caused the regrowth.

Some research suggests that biotin supplements may be of most benefit in people who have a deficiency of the nutrient; however, again, there is a lack of studies that have measured biotin levels before and during supplementation to confirm this conclusion.
