Most studies have not shown a correlation between fat-free mass and LBP, while one study has demonstrated a negative association but only in women [ 34 ].

The android fat mass relative to gynoid fat has a greater correlation with LBP [ 22 ]. The results of this large cross-sectional study are inconsistent with those of previous studies. No significant associations were found between all measures of fat mass, fat-free mass, and LBP after adjusting for confounders.

To the best of our knowledge, only one study has shown similar results to those of previous studies. In that study, it was concluded that the correlation between obesity, fat mass, and LBP disappeared after adjusting for genetics and early environmental factors shared by twins [ 39 ].

Recent research has been focused on investigating various factors while adjusting the higher incidence of LBP in older adults and women, as previously revealed [ 40, 41 ]. A study reported that adverse risk factors of LBP included characteristics of the individual, such as older age [ 10 ].

On the contrary, another study has demonstrated that LBP was more common in women than in man, and declined with increasing age [ 17 ].

According to these studies, age does seem to have effect on LBP. Therefore, this study was adjusted by age as the confounding factor. Generally, acute back pain is defined as lasting less than 4 weeks, subacute back pain lasting for 4—12 weeks, and chronic back pain lasting for more than 12 weeks; the questionnaire that we used for this study includes only back pain lasting for more than 30 days.

Therefore, we were not able to include the data on acute and subacute back pain. Patients with chronic LBP were defined as individuals in this survey who had complained of LBP for more than 30 days during the past 3 months. This study has a few limitations.

The analyzed LBP was nonspecific not classified as disc herniation or spinal stenosis. Although disc herniation, spinal stenosis, and back muscle degenerative change are regarded as important factors to be considered in LBP in several studies [ 5, 7 ], our study does not present such data. Additionally, it was not possible to analyze the characteristics of back pain, such as whether or not it was accompanied by radiating pain.

This study is contrary to previous studies that concluded that there is a correlation between obesity and fat mass and LBP. This shows that statistical significance cannot be found when risk factors for LBP and age and sex that can become compounders are adjusted.

It is worth noting that the results of this study are contrary to those of previous studies, as the analysis was conducted on a large number of participants.
